In Treatment Plan Template and Program Setup, you can view Treatment Plan Grids and Diagrams.

View the Goal-Objective-Intervention Grid

1. Navigate to Treatment Plan Template and Program Setup by following the path above.
2. Click the To view the Goal-Objective-Intervention Grid, Click Here link. The page displays which Goals are linked to which Objectives and Interventions in alphabetical order.

1. Click the Return button to go back to the Treatment Plan Template and Program Setup page.

View the Goal Measurable Diagram

1. Navigate to Treatment Plan Template and Program Setup: Main Menu > Admin Tools Menu > Treatment Plan Template and Program Setup.
2. Click the To view all the Goals with their Measurables, Click Here link.
3. The page displays which Outcomes are linked to which Goals and Objectives in alphabetical order.

• Outcome Titles are denoted by the following text format: OUTCOME
• Goal Titles are formatted as the following: Goal
• Objective Titles appear in bold text: Measurable

1. Select one of the following options:

• Click the PCP Setup button to return to the Treatment Plan Template and Program Setup page.
• Click the Main Menu button to return to the Main Menu.

View the Outcome-Goal-Objective Diagram

1. Navigate to Treatment Plan Template and Program Setup: Main Menu > Admin Tools Menu > Treatment Plan Template and Program Setup.
2. Click the To view the Outcome-Goal-Objective Diagram, Click Here link. The page displays which Outcomes are linked to which Goals and Objectives in alphabetical order.

• Outcome Titles are denoted by the following text format: OUTCOME
• Goal Titles are formatted as the following: Goal
• Objective Titles appear in bold square-bulleted text: Objective
• Item descriptions are printed in gray-colored text: Description

1. (Optional) Click the Select Program drop-down to review a specific program. By default, all programs display.
2. Select one of the following options:

• Click the PCP Setup button to return to the Treatment Plan Template and Program Setup page.
• Click the Main Menu button to return to the Main Menu.

View the Intervention-Goal-Objective Diagram

1. Navigate to Treatment Plan Template and Program Setup: Main Menu > Admin Tools Menu > Treatment Plan Template and Program Setup.
2. Click the To view the Intervention-Goal-Objective Diagram, Click Here link. The page displays which Interventions are linked to which Goals and Objectives in alphabetical order.

• Intervention Titles are denoted by the following text format: INTERVENTIONS
• Goal Titles are formatted as the following: Goal
• Objective Titles appear in bold square-bulleted text: Objective
• Item descriptions are printed in gray-colored text: Description

1. Select one of the following options:

• Click the PCP Setup button to return to the Treatment Plan Template and Program Setup page.
• Click the Main Menu button to return to the Main Menu.
